PUTTIN’ ON THE HITS ENTRY FORM

Choose your favorite artist, band, or group and perform LIVE ON THE THOMPSON PAVILION STAGE, Friday, September 30, 2011 at 8:00 p.m.

This is a LIP SYNC CONTEST, that gives you a chance to win $$$ MONEY $$$!

Have fun acting out your favorite song and be a part of the Annual Spring Valley Potato Festival. 

Be sure to:

·          Dress the part

·          Bring your own CD of the 3-4 min song you wish to perform.  NOTE: SONGS MAY NOT BE LONGER THAN 4 MINUTES

NO PROFANITY OR SEXUALLY EXPLICIT/SUGGESTIVE MATERIAL ALLOWED,  OR YOU WILL BE DISQUALIFIED

·          Complete the entry form below and bring the form to (any of the contacts listed below) at the stage Friday evening by 7:00 p.m. Forms will be available at the Thompson Pavilion at 6:30pm. NOTE: FORMS MUST BE RECEIVED BY 7:00 P.M.

You may perform as a solo or group, but a group awarded a cash prize must share their winnings.  Only one song may be performed.  NOTE: EACH PERSON MAY ONLY PERFORM IN ONE ACT.

Prize categories are in two groups:  6th grade & under and 7th – 12th grade.

No one over 12th grade will be eligible for entry.

Judging will be on:        

1st prize  $50.00               2nd prize  $25.00           3rd prize  $15.00
